Applicant Instructions

Please review before beginning your application:

  • Copy of DD-214 (discharge papers - hide sensitive information)
  • Medical provider statement confirming disability and need for a service animal
  • Copy of government-issued photo ID
  • Proof of residence and pet policy (if applicable)
  • Two non-family references’ contact information

Applications are typically reviewed within 14 business days. You will be contacted to schedule an interview after initial review.

Estimated total cost per service dog is approximately $20,000–$30,000. Recipients typically contribute or fundraise 10–20% ($2,000–$5,000). All funds are processed by All American K9 for Vets Inc.; the organization covers the remainder via grants and donations.

Information for Applicants

  • Travel may be required for training and placement; limited assistance may be available.
  • Until completion of training/transfer, dogs remain property of All American K9 for Vets Inc.
  • Recipient is responsible for ongoing routine care (food, supplies) and veterinary costs after placement.
  • Please promptly report changes affecting the dog’s welfare (housing, health, etc.).
  • Noncompliance with care, safety, or training expectations may result in program separation.
